Access Code
 


This page helps you set up an access code to organize data you are entering online.  This page is part of the four step process required for drafting a memorandum of understanding for modification of the Medicaid health plan. 

1. What is your email?    This information is used to contact you if we notice a problem with the data you are supplying.

You decide about this code.  This is a six character alphanumeric code that will be required for entering data. Since you enter data at different times and using different forms, the code is used to link your data within our database You will also need it to receive reports based on your data. Please keep your code in a safe place (we will not be able to provide you with the code if you forget it.
