1. List the initials of your friends and family members in order of their frequency of contact with you. 

If two people have the same initials, follow the initials with numbers so that they can be distinguished from each other.  List the person you have the most contact with first.  Leave blank if you have listed most of the people you have daily, weekly or monthly contact with.

2. List the initials of your friends and family members in order of their importance to you. 

Use the same initials you used to answer the question on frequency of contact.  You do not need to list all the people you have contact with.  List the person whose view is most important to you first.  Leave blank if you have listed most of the people who are important to you.  List in order of importance.

3.  For each initial you have entered, check if the person has a positive or negative attitude towards drug use.   

All contacts not listed are assumed to have a neutral influence on your drug use.
