Importance of Contact

This is the second question in the Orientation of Social Support instrument.  Complete all three questions and you will receive automated scoring regarding the influence of your social network on you.  No patient identifying information is collected or should be provided. 

Please note that you will need to use the same code you used for entering your data in response to the question on Frequency of Contacts.  If you have forgotten this code, please start from the beginning and reenter your data.  We provide no method of retrieving forgotten codes.

1. List the initials of your friends and family members in order of their importance to you. 

If referring to the same person, use the same initials you used in the previous questions.  List the person you consider most important to you first (Rank 1).
