Frequency of Contact

This is the first question in the Orientation of Social Support instrument.  Complete all three questions and receive automated scoring regarding the influence of your social network on you.  No patient identifying information is collected or should be provided. 

Please note that you will need to select a code for entering your data.  If you forget this code, you will lose access to your data.  We provide no method of retrieving forgotten codes.

1. List the initials of your friends and family members in order of their frequency of contact with you. 

If two people have the same initials, follow the initials with numbers so that they can be distinguished from each other.  List the person you have the most contact with first.  Leave blank if you have listed most of the people you have daily, weekly or monthly contact with.
